W-9, Vendor Forms, and Other Procurement Documents

Before a district can cut a purchase order, its business office usually has to set Education Walkthrough up as a vendor. That means a W-9 and often a vendor application. Here is what we have ready and how to get it.

What we can send

  • Form W-9. Signed and current. Education Walkthrough LLC, Chicago, Illinois.
  • Vendor application forms. Send us your district’s form and we return it completed.
  • Certificate of insurance. General liability and workers compensation, on request.
  • Sole-source letter. For districts whose policy allows a sole-source purchase when no equivalent product exists.
  • Data privacy agreement. We sign your district’s DPA or provide ours, along with our data security and privacy documentation.

How to request them

  1. Submit a ticket. Say which documents you need and the district name.
  2. Attach your vendor form or DPA if you have one. If the form needs a quote number or a dollar amount, generate the quote first and include it.
  3. We return the documents by email. Most requests are answered within a business day or two.

Frequently asked questions

Q: Can I download the W-9 myself?
A: Not yet. Ask and we send it. Documents are shared on request so the business office always gets the current version.

Q: We already have you as a vendor. Do we need a new W-9?
A: Only if your office asks for an annual refresh. Our details have not changed.

Q: Where does the PO go once it is approved?
A: Upload it to your quote in the dashboard, see Paying by Purchase Order. Licenses activate when we approve it.

Q: Our office needs an invoice before paying.
A: Invoices come from our accounting system after the PO is approved, and are attached to your quote under Billing where you can download them.
